School Entrance Forms
Directory Information Form
This form should be completed for all families new to Alpha School. The information provided on this form will be included in our directory and the e-mail addresses you provide will be used for our weekly e-mail blasts.

APEX Forms
APEX Agreement and First Aid Authorization Agreement - Before a child can attend APEX parents/guardians are required to complete an APEX agreement and First Aid authorization agreement.
Medical Forms
Physical Exam Form K-8
This form should be completed by your child's Primary Health Care Provider for the following school years:

Dental Exam Form
This form should be completed by your child's Dentist for the following school years

Allegheny County now requires that all children be tested for lead exposure prior to entering kindergarten.
Forms For Medication Administration at School
Allergy Form and Allergy Waiver
This form must be completed by you and your child's physician indicating the steps to be taken if your child has a life-threatening allergy. It's important to note that if Benadryl is part of the plan, the physician should include the written prescription for that medication.
Medication Administration Form
This form must be completed by you and your child's physician when requesting any medication be dispensed to your child during the school day.
Indemnity Form
This form must also be completed by you when requesting any medication be dispensed to your child during the school day.
Attendance Forms
Attendance Form
This form is to be completed if your child misses school.
Absence Request Form
This form is to be completed if you know in advance that your child will be missing school. Whether it be for one day or for an extended period of time, we ask that you still complete the form. For example, if they are going away on vacation or going on a shadow day for high school.
Sports Forms

This form must be completed by you and your child's physician before participating on any Athletic team.
Volunteering Forms

This form will help parents complete the necessary clearances required in order to volunteer at the school.

Bus Information Form
This form must be completed prior to school starting to ensure your child is registered for bus transportation and to inform the school of the bus number and bus procedures for the district.
